Clinical Dementia Rating (CDR)
Purpose: Clinically stage the severity of cognitive-functional impairment in dementia--rates the severity of dementia
Severity = none, questionable, mild, moderate, severe.
Admin Time: 40-75 min
User Friendly: Allow at least 2.5 hrs of preparation for the first assessment
Administered by: Interviewer administered semi-structured interview of patient and carer.
Most appropriate: Comprehensive assessment. Acute, Primary, Community, and Residential.

Dementia Severity Rating Scale (DSRS)
Purpose: Multiple choice questionnaire for caregivers to assess the mildest to the most severe of Alzheimer’s Disease and other dementias in the major functional and cognitive domains.
